    1. Apparently, cannot save document as PDF - the universally accepted document format. 2. Cannot rotate view from portrait to landscape. 3. Apparently, cannot save document to any location other than company using form. 4. Last step is annoying and intimidating: “Are you ready to send form?” Are you serious? This sounds like the IRS asking if this is your “final answer” in their version of Jeopardy. This is especially true since a customer apparently will not have been able to retain a copy - preferably a PDF - of what he/she has just completed and is sending! Scary! 5. Above deficiencies may work for company using form, but they definitely do not work for this customer. 6. Do NOT recommend Form Filler. I’ll take an Adobe Fillable PDF any day!

    no instructions on the birthday date format and I could not submit form until the 5th try where I finally saw the date format on today’s date auto generated

    This app would be good if you could enter the data a little easier. When you using your phone to fill this out it is very difficult to get to the right lines and your prompts cover what you need to fill out. Always asking if you want to exit because you tapped outside of the field. What should have taken less then 5 minutes took almost 20 .

    App doesn’t properly display. Same as using the phone browser. Defeats the purpose of having an app. Glitchy as well. Kept asking me to click “yes” when “no” was already checked.
